After you select Find, Diia can display the type of medical conclusion, creation information, document number, the healthcare institution that issued the conclusion, and the period for which it is valid.

For a medical conclusion on childbirth, the online form requires:

• the medical conclusion on childbirth number;
• the mother's RNOKPP;
• the mother's passport series and number or ID-card number.

The returned information can include the type of medical conclusion, information about the mother, document number, issuing authority, issue date, child's sex, place of birth and date of birth.

No. A medical conclusion on temporary incapacity for work and an electronic sick-leave certificate are separate electronic documents.

The treating doctor records the medical assessment by creating the medical conclusion on temporary incapacity in Ukraine's electronic healthcare system. For an insured person, information from that medical conclusion is then used for the electronic sick-leave process. The doctor does not personally create the electronic sick-leave certificate.

A medical conclusion on childbirth is an electronic document created from medical records in Ukraine's electronic healthcare system. It records the doctor's conclusion confirming the fact of the child's birth and can serve as a basis for registering the birth with the state civil-status registration authorities.

What should you check if Diia does not find the conclusion?

Start by checking the information entered in the form against the details you received for the medical conclusion. A mismatch in the conclusion number, RNOKPP or passport or ID-card details can prevent the correct record from being identified.

For a temporary-incapacity conclusion, make sure that the conclusion number is entered in Latin characters. If the identifying information is correct but the expected medical record is still unavailable, contact the healthcare provider that created the medical conclusion so it can check the underlying medical record and patient information.
