How to Apply for State Support When Caring for a Seriously Ill Child in Ukraine

Who can receive this caregiver benefit?

This state assistance is specifically for a family caring for a seriously ill child who has not been granted disability status. It is not a general payment for every childhood illness.

One of the following people may receive it:

• a parent;
• an adoptive parent;
• a guardian or custodian;
• one foster parent;
• a foster-family educator.

The applicant must permanently live with and provide care for the qualifying child. Only one eligible carer receives the benefit for the same child.

Which illnesses and conditions qualify?

Ukrainian law restricts this benefit to children with specified serious illnesses, injuries or conditions. The statutory categories include:

• serious perinatal damage to the nervous system;
• serious congenital malformations;
• rare or orphan diseases;
• oncological diseases;
• oncohematological diseases;
• cerebral palsy;
• serious mental disorders;
• type 1 insulin-dependent diabetes;
• stage IV acute or chronic kidney disease;
• serious injury;
• a need for organ transplantation;
• a need for palliative care.

The detailed medical list is established by Cabinet of Ministers Resolution No. 1161. A diagnosis must be confirmed through the prescribed medical process; describing a child's illness as serious without the required medical certificate is not sufficient for this benefit.

What medical certificate do you need?

The central eligibility document is Form No. 080-3/o, a certificate concerning the child's qualifying illness or condition. It is issued by the medical advisory commission of the healthcare institution under the procedure established by the Ministry of Health.

The certificate confirms the qualifying condition for a child who has not been granted disability status. If the child already has established disability status, this particular benefit is not the correct payment.

What documents must you submit?

The Pension Fund's 2026 administrative service card requires the applicant to establish identity and provide the information and documents needed to verify eligibility. The core package includes:

• the application form approved by Cabinet Resolution No. 765;
• an accepted identity document;
• the applicant's RNOKPP taxpayer registration information, subject to the statutory religious-refusal exception;
• a copy of the child's birth certificate;
• Form No. 080-3/o issued by the medical advisory commission;
• where applicable, documents confirming adoption, guardianship, custodianship or placement in a foster family or family-type children's home.

If information can already be obtained or confirmed through Ukrainian state electronic registers, the applicant does not have to submit the same document again where the current rules allow electronic verification.

Where can you apply?

The Pension Fund of Ukraine administers this state assistance in 2026. Current official guidance allows applications through several channels:

• a Pension Fund service center;
• an authorized executive body of a village, settlement, city or city-district council;
• a CNAP administrative service center;
• by post to a territorial body of the Pension Fund;
• electronically through the Diia Portal, including the Diia app, where technically available.

The availability of an electronic submission channel does not remove the medical eligibility requirement. The Pension Fund must still be able to verify the required Form No. 080-3/o and the other information necessary for a decision.

How much is the benefit in 2026?

The monthly benefit equals two subsistence minimums for children of the corresponding age.

Ukraine's 2026 State Budget sets the subsistence minimum at UAH 2,817 for children under 6 and UAH 3,512 for children aged 6 to 18. The resulting monthly assistance in 2026 is therefore:

• UAH 5,634 per month for a child under 6;
• UAH 7,024 per month for a child aged 6 to 18.

The benefit is awarded separately for each qualifying seriously ill child and is paid regardless of receipt of other types of state social assistance for children.

When does payment start and how long does it last?

The benefit is assigned from the date of application. It is normally granted for a period of six months.

This makes the application date important: the ordinary rule does not provide retroactive payment for months before the claim was made merely because the qualifying illness existed earlier.

How do you extend the payment for another period?

If the child remains eligible, payment for a further period is continued on the basis of a new application from the child's legal representative and a current certificate confirming the child's illness.

If the renewal application is received within one month after the previous payment stops, the continued payment can be assigned from the date on which the previous payment stopped. Missing that one-month window can affect the effective date of the renewed award.

How long does the application take?

The current Pension Fund administrative service card states that the application must be considered no later than 10 days after the complete application, documents and required information are received.

If required documents or information are missing, the Pension Fund can ask the applicant to provide them. The 10-day decision period therefore should not be treated as running from an incomplete filing when essential evidence is still outstanding.

What if you submit an incomplete application?

If required documents or information are missing, the Pension Fund informs the applicant what must still be provided. Current administrative guidance provides a one-month opportunity to supply requested missing materials in relevant cases while preserving the original application date.

Check the birth certificate, legal-representative documentation and Form No. 080-3/o before filing to avoid delaying the decision.

When can the benefit be refused or stopped?

The Pension Fund's current service card identifies grounds that can prevent an award, including:

• failure to establish the applicant's identity or authority;
• failure to complete required electronic identification for an electronic claim;
• the child has been granted disability status;
• the child has reached age 18;
• the child has been placed on full state maintenance;
• an adoption decision has been cancelled or declared invalid;
• a guardian or custodian has been released from their responsibilities;
• the benefit has already been granted to another eligible carer who permanently lives with and cares for the child;
• false information is identified in the submitted documents.

If disability status is established while this benefit is being paid, payment continues only through the day disability status is established. The family should then check the benefits applicable to a child with disability rather than relying on this specific no-disability-status payment.

This benefit is separate from other child assistance

The seriously ill child caregiver benefit addresses a narrow situation: the child has one of the qualifying serious conditions but has not been granted disability status. It does not replace ordinary family benefits or benefits based on different eligibility rules.

Common mistakes to avoid

• Assuming any serious diagnosis automatically qualifies without checking the statutory list.
• Applying without Form No. 080-3/o from the medical advisory commission.
• Using this procedure after the child has already been granted disability status.
• Failing to provide documents confirming adoption, guardianship or foster-care authority when they apply.
• Waiting until long after the medical certificate is issued and assuming payment will automatically be backdated; the benefit normally starts from the application date.
• Missing the one-month renewal window after a six-month award and expecting uninterrupted continuation from the previous stop date.
• Submitting a second application when another eligible carer already receives the benefit for the same child.
